Tell us if you are living with dementia

If you or a loved one are living with dementia, please tell us so that we can update our records. We can make sure that we make reasonable adjustments or small changes to the way we care for a person living with dementia to ensure that they have the best possible experience when visiting Moorfields Eye Hospital.

This is me booklet: The this is me booklet from Alzheimer's Society provides extra support for people living with dementia when coming to hospital. Please download a copy or ask staff for one at your next visit.

Helping hands sticker: A helping hands sticker can be stuck to your patient notes and helps us to identify people who may have extra needs or require reasonable adjustments to their care. 

John's campaign: In January 2017 Moorfields signed up to John's Campaign to pledge our commitment to supporting the carers of patients with dementia. The aim of the campaign is to encourage and support carers, which includes allowing carers to stay with their loved ones in hospital (where practicable) and allowing carers to be involved in the care provided to patients in hospital if they so wish. We recognise that patients with dementia will have a better patient experience and improved recovery time if a person they are close to stays with them and provides support to them in hospital.

We have a range of easy read patient information leaflets that you might find helpful.

In addition, SeeAbility has created easy read factsheets and information leaflets on a number of eye conditions that people living with dementia are more likely to experience.
